Complete the online OCPS Registration Application (instructions below) then send it along with the necessary documentation to [email protected].
-Your government issued ID
- Birth Certificate
- Immunization Form 680
- Physical Exam
- Verification of Address (Homestead Exemption Card, Property Tax Statement, Signed Settlement Statement or Current Lease)*
- IEP/504 if applicable
*If the parent or guardian does not have a lease or proof of home ownership in his/her name they will need a notarized statement issued by the Office of Student Enrollment. To make an appointment with the Office of Student Enrollment to verify residence. There you will find instructions on how to make an appointment along with the documentation needed.
